LuaEntity Belongs to Surface

I have a headless server with multiple surfaces. I get the screenshotted error message. I have uploaded the save file. It seems to be repeating. No consequence of the bug noticeable. Wondering what's happening?

This is almost certainly due to a Mod incorrectly modifying a LuaEntity somewhere. The two most likely candidates in the Save file are Teleporters and Grappling Gun.

I played around with teleporting between surfaces as well as using the grapple - nothing interesting happened. If you can figure out how to reliably cause the Error to appear it would be possible to find the bad line of code in the Mod.

I was playing more and noticed the bug again.
I got it to repeat.

I added a new save file attachment. From my position on Fulgora with the new save. Press M then view Nauvis.
It plays the bug then.
